Up to $185,000 total annual compensation (salary + bonus)
Quartet Health seeks Family Nurse Practitioners to provide holistic care for patients with serious mental illness in New Jersey. As a critical member of our multidisciplinary team, you'll manage whole-person care and treatment for adult patients, both virtually and in-home.
- Serve as the "Quarterback" of the Care Team
- Conduct initial intake visits and develop individual care plans
- Prescribe and manage medications, including injectables
- Collaborate with a broader team for comprehensive patient care
- Provide care both in-home and via telehealth
- Active nurse practitioner license and certification in New Jersey
- Expertise in treating chronic and urgent medical/psychological conditions
- Ability to travel locally at least 75% of the time
- Valid driver's license, active auto insurance, and access to a car
- Excellent communication skills and experience with Electronic Health Records (EHR)
- Willingness to assist with occasional after-hours call coverage
- Experience in home-based services or complex care cases
- Dual FNP / PMHNP background
- Bilingual (Spanish and/or other languages)
- Telehealth experience

$145,000 - $160,000 base salary/year (plus 15% annual target bonus)
