$145,000 - $160,000 /year (plus 15% target bonus)
Quartet Health is seeking Family Nurse Practitioners to provide holistic care for patients with serious mental illness. NPs will manage whole-person care and treatment for adult patients, both virtually and in-home, as part of a multidisciplinary team.
- Work involves both in-home and virtual care
- Serves as the "Quarterback" of the Care Team
- Conducts initial intake visits, develops care plans, and manages medications
- Collaborates with a broader team for comprehensive patient care
- Active nurse practitioner license and certification in New Jersey
- Expertise in diagnosis and treatment of chronic and urgent medical/psychological conditions
- Ability to travel locally at least 75% of the time
- Valid driver's license, active auto insurance, and access to a car
- Excellent communication skills and experience with Electronic Health Records (EHR)
- Willingness to assist with after-hours call coverage
