$42.31 to $83.27 per hour (New York Residents)
Description:
Optum Home & Community Care, part of UnitedHealth Group, is focused on building an integrated care model that addresses patients' physical, mental, and social needs. This includes providing care in nursing homes, assisted living facilities, and senior housing. The role involves primary care delivery, managing chronic and acute conditions, performing comprehensive assessments, and coordinating care across different settings.
Qualifications:
- Certified Nurse Practitioner (NP) or Physician Assistant (PA) through a national board
- Graduate of an accredited master's degree in Nursing (MSN) program for NPs or accredited Physician Assistant degree program for PAs
- Active and unrestricted license in the state of residence
- Active DEA licensure/prescriptive authority (or ability to obtain post-hire)
- Reliable transportation
- Ability to work 24 hours per month with availability during off-hours
Preferred Qualifications:
- 1+ years of experience in Long Term Care
- Understanding of Geriatrics, Chronic Illness, and Advanced Illness
- Proficient in computer skills and electronic medical records
- Ability to develop and maintain positive customer relationships
