Oak Street Health is seeking a Nurse Practitioner to provide value-based primary care to adults on Medicare. This role involves working independently and collaboratively with physicians to keep patients healthy and out of the hospital.
- Schedule: Full-Time, Monday through Friday, 8:00 AM - 5:00 PM
- Practice in outpatient settings as part of a collaborative care team
- Focus on primary care, care coordination, and population health
- Conduct office visits, annual wellness visits, and health risk assessments
- Participate in care coordination and population health leadership
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in Family NP, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
- Experience in primary care: internal medicine, geriatrics, or family medicine
- Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments
- Passion for teamwork and cross-functional collaboration
- Bilingual proficiency (in applicable areas)
