  • Experience pathways

    Standard path: 2 years as a perioperative RN. Alternate path: 18 months with an eligible CFPN, CST, TS-C, or military equivalent, plus the same hour floors.

  • Hour floors

    2,400 total perioperative RN hours, including 1,200 intraoperative hours. Current full- or part-time perioperative work is also required.

  • Exam & fees

    200 items (185 scored) over 225 minutes. Passing scaled score 620. Application $475; retake $175.

  • Renewal

    5-year cycle with 500 perioperative hours (250 intraoperative impact) and 300 activity points. Recertification fee $400.

What experience is required for CNOR?

The standard CCI pathway requires at least two years and 2,400 hours of experience as a perioperative RN, including at least 1,200 intraoperative hours. A published alternate pathway uses an 18-month duration for eligible CFPN, CST, TS-C, or military-equivalent holders, while retaining the hour requirements.

Must I currently work in perioperative nursing?

Yes. CCI requires current full- or part-time work in perioperative nursing clinical practice, education, administration, or research at application.

How much is the CNOR application?

CCI's May 2026 handbook lists a $475 application fee including the first exam attempt. The current public page lists a $175 retake fee. Fees and testing policies can change.

What is the CNOR exam format?

The exam contains 200 multiple-choice questions, with 185 scored and 15 pretest questions, and allows three hours and 45 minutes. CCI reports a passing scaled score of 620 on a 200-800 scale.

How often is CNOR renewed?

Current CCI rules use a five-year cycle. Renewal requires an active credential and RN license, current perioperative work, 500 perioperative hours with 250 impacting intraoperative care, and 300 professional activity points. CCI does not currently offer renewal by exam alone, and CE/NCPD alone is not enough.
